I've had this thing for 30+ years. Got it at a Garage Sale best I can remember around 1985. I just did some research on it and found the Harmony Database. From that I found out it is the Model # "H-6 Tone Emphasizer" with a Gibson P-13 pickup. This model was produced in 1954 only, so it's 62 years old. Here is the link to the Harmony Database on it:

I only recall playing it live with a band only a couple of times over the years. I have done a lot of recording with it over the years when I felt a song needed a steel part like on this instrumental of "Beulah Land" where I used it for some fills and also a lead part right after the harp lead.

I play it with the bridge/pickup cover and also the tone knob removed. Just put the cover and knob on to take pictures.
So if anyone else likes Lap Steals, has some pictures or recordings--please share. Platefire
