every schematic i've come across have the reverb driver and recovery triodes on different tubes. is there a design reason for this or just the way the layout of a circuit came to be? Any issue having the reverb driver and recovery using the same tube? Ex. driver V4A and recovery V4B

It's happenstance, and driven by where you're looking.
Plenty of folks in this forum have used a "one tube reverb" where the driver and recovery are in the same envelope.
Ampeg did onboard reverb in their Reverberocket amp (https://el34world.com/charts/Schematics/files/Ampeg/Ampeg_r12r_reverb_rocket.pdf) before Fender. There is a 6SN7 (V3) that does drive and recovery in a single envelope.
