Audio worker electrocuted

Electrocution is rare in audio. But it happened Friday, setting up audio for a race in Chicago.

Duane Tabinski .... a witness heard him groan and found him “slumped over” .... Tabinski had been hired by NASCAR to produce audio for pre-race celebrations. The witness went to help, but another worker stopped him because Tabinski, 53, was in a “padlock” area, meaning high-voltage electricity was circulating,..."

I know anything could have happened, but obviously not a rookie mistake... Faulty equipment? I've seen situations where over confidence leading to neglect becomes and issue...
